In Dec 2008—14 year old male child was brought with the complaints of recurrent upper respiratory tract infection which is once /month since many years. He will have throat pain with high fever with nose block for which he has to take frequent allopathic treatment.
Along with that mother complained that since 6 month his voice is becoming hoarse, he has put on weight around 10 kg (from 35 to 46). At present his weight is 44 kg.He has slowed down in his activities. Becoming more irritable and cranky. Memory is good. There are no other complaints.
There is a family h/o hypothyroid in his brother from 12 yrs of age who is on treatment. There is also family h/o hypothyroidism in maternal grandmother.
He has dry rough whitish plaques on knee and ankle with itching since 2-3 yrs.He had complaints of skin eruption on buttocks and thigh which were diagnosed as fungal and treated with local application and medication 6 month back
Considering the family history and present symptomatology child was investigated for thyroid and REPORTS WERE IN FAVOUR OF HYPOTHYROIDISM WITH TSH –15.75 (N—0.30-5.5) and free T4 towards lower side but within normal limits.
Patient is on treatment since 3 yrs his last reports were done in Nov 2010. This shows TSH-6.15 and FREE T4—1.01. This shows thyroid normalcy.
CLINICAL – HIS GROWTH IS NOT AFFECTED. He is doing well academically. His height and weight are in consistency with the normalcy. Skin no complaints and throat infections are very occasional. He is still on treatment.
